Index Gear Ratio = Index Constant / Teeth
Feed Gear Ratio = Table Feed / M
where M is inch of table travel per revolution of workpiece. Feed Constant, M, is likely a decimal value (.075, .0375, for example). If the machine is metric, then M would likley be mm of table travel per revolution.
Or sometimes Feed Constant K appears as follows:
Feed Gear Ratio = K x Table Feed
where K is revolutions of workpiece per inch of table travel. K is most often expressed as an integer (6, 8, or 10 for example). However, if machine is metric, then feed is likely a decimal value.
