You did the extra idlers and doubled an index driver gear correctly, else your tracking test would have failed.

Did it strip teeth from the cutter? I think that's what you meant to say...

What size machine is it? what is the pitch?

I could see it happening if the cutter jerked the table under the workpiece while taking up table drive backlash - something to look for when climb hobbing.
