First, check the table angle. This is the angle you set the cutter table to which should be helical angle +/- cutter angle.
Next, check end-play of the cutter arbor with cutter installed. There should be nearly zero end-play.
Also, the setup itself, for high helix angles, can sometimes back drive the index gearing.
This will cause all kinds of problems due to backlash in the index gearing (See article in link below).
Since you have been investigating everything else, this might be your problem.
